Travel Endorsement Request/I-20 Reprint

Important Information:

This form is intended for F-1 and J-1 students who require a travel endorsement on their Form I-20 or DS-2019. If your visa is not sponsored by the University of Memphis (such as, Fulbright), please contact your visa sponsor to provide you your travel endorsement.
F-2 or J-2 dependents should not fill out this form. The primary visa holder F-1 or J-2 should submit the form.
F-1 and J-1 enrolled students (as well as their dependents on F-1 and J-1 visa status) need a travel signature on their Form I-20 or DS-2019 no older than 12 months upon entry to the U.S. with their valid F-1 or J-1 visa.
Students under OPT or STEM OPT need a travel signature no older than 6 months upon entry to the U.S. with their valid F-1 student visa.
