I hereby certify that my job is directly related to my major field of study and that I am working more than 20 hours per week.

I understand that any change in my residential address, phone number, employer, termination of employment, or any other change in employment MUST be reported to ISS or the SEVP Portal within 10 days of the change.

I understand that I must report/confirm my employment to ISS every 6 months. It is my responsibility to complete the self-evaluations on the I-983 form and submit a copy to ISS at the 12-month and 24-month mark in my STEM OPT authorization period.
