This form will be used to request Approval/Send of a Slate communication. It is also to be used to report all issues with Slate, including but not limited to Student Records, Forms, Queries, Workflow, Reader, Events, Communications, and any other problem. To request/modify Slate Access, please fill out the Slate User Access Request form. To request a communication be created by a Slate Admin, please fill out the Communication Request Intake form. To request a new project be built in Slate, please fill out the Slate Project Request form to be reviewed by the Slate Steering Committee.
